New Client Forms

New clients must complete intake forms prior to the start of the first session*.

For your convenience, you may complete the necessary registration forms at any time before your first appointment. This will save you time when you arrive in the office, and will not cut into your allotted session time. Please keep in mind that it will take a fair amount of time to complete the forms, as the Information Questionnaires for Adults and Minors include symptom checklists and personal history questions.

The forms are in .pdf format, which you can read and fill out using Adobe Reader.

You may complete and submit the forms in any one of these ways:

  1. Request a secure (encrypted) email from me that contains the necessary forms. I will then send them to you as an attachment. Open the attachment and save it to your computer, using your last name as the filename. Fill out the forms on your computer using Adobe Reader, save the completed file to your computer, then send them to me as an attachment via email. IMPORTANT: to protect your privacy, you must use the “Reply” feature contained within the secure email you received from me. If you choose to send them attached to an unencrypted email, your privacy may not be secure. –OR–
  2. Request a secure (encrypted) email from me that contains the necessary forms. I will then send them to you as an attachment. Open the attachment and save it to your computer, using your last name as the filename. Fill out the forms on your computer using Adobe Reader, save the completed file to your computer, then print out the forms you have filled in, and bring them with you to your first appointment. –OR–
  3. Download the packet of forms, fill out the forms on your computer using Adobe Reader, save the completed file to your computer, then print out the forms you have filled in, and bring them with you to your first appointment. This is the packet for new ADULT clients (age 18+). This is the packet for new CHILD clients (under age 18) –OR–
  4. If you are unable or do not wish to complete these forms on the computer, you will be provided with blank forms that you can fill out by hand at the office, prior to your appointment. Please note that if you cannot make arrangements with me to come in to the office approximately 30-45 minutes before your appointment time, the time it takes to complete the forms will be the first part of your scheduled session.

* New clients who are using EAP benefits do not need to complete the same forms as clients who are using standard insurance benefits or who are paying out-of-pocket. Because EAP benefits are very limited, and require different forms, EAP clients will be asked to complete only a few forms prior to their first appointments.
